: Hello: : I am having the same problem with Washignton Insurance Company. I too received mail from them regarding a policy that I was unaware existed. They want me to sign an Opt Out Request form. However, I am trying to find out about the policy. There doesn't seem to be any info to contact them other than the PO Box in Carol Stream Illinois. If you have any more info on this please let me know. Thank you.
