: To whom it may concern: My name is Susan Ward and I work for Ransom Memorial Hospital in Ottawa, KS. I have had various products from AFLAC. However, for some reason I received a letter stating that I have to reinstate my coverage for short term disability. My policy number is P0728433 and my account number is 25899. I have continued to have the premiums taken out of my pay check. Why has this policy lapsed????????????? Please provide me an answer. I have attempted since 12/2/02 to call you at your 800 number and the line is busy constantly. : Thank you for your prompt attention. Susan
