The form i was telling you about is a form to change a hmo to a ppo and on that same form they have a section for her father to put the beneficiary and it has the girlfriends name. Have you ever heard of fort dearborn insurance company? Also the letter that the insurance company wrote to the state said that ger father change the beneficairy on september 2, 2002 but in a letter from the company that was srent to my attorney at the time they said he change it on the september 8 2002 we have both letters. They have never produced a form that shows her father change the beneficiary, any other advice?
