: I have seemed to misplaced my life insurance policy. I bought it approximatly 15 years ago. I don't believe I even changed my name on it. I need to change my name and I don't know who to contact. I would also like a copy of my life insurance policy. I have monthly payments coming out of my checking account, that has never changed.
